When a patient needs a copy of his or her medical record, the 150 employees and 30 contractors who work for the Barnes-Jewish Hospital Health Information Management Department are available to help. The department is open Monday through Friday from 8 a.m. to 4 p.m. Health Information Management is located on the first floor of Barnes-Jewish Hospital South campus and the first floor on the North campus.

To obtain a copy of a medical record, call or write the HIM Correspondence unit. Please submit a valid authorization form signed and dated by the patient/legal guardian. To avoid a delay, please make sure to answer all questions on the form.

The authorization form is in PDF format and requires Adobe Acrobat Reader. If you don't have this software, go to Adobe for a free download.
