Organ Donation and Transplantation Documentary Opt-Out Request

The Washington University and Barnes-Jewish Transplant Center has been approached by the A & E Cable Channel to film a documentary television program highlighting the organ transplant process by following organ donors and organ recipients. We feel this program will provide an opportunity to raise organ donation awareness and show how recipient lives are changed when a family decides to donate.

There is a chance you may be asked if you would like to voluntarily participate in the filming if your transplant takes place over the next several months. A member of the Barnes-Jewish or Washington University School of Medicine media relations team may contact you in advance of your surgery to ask permission and will explain the process in detail. This would involve a film crew meeting you before surgery, videotaping your transplant surgery and interviewing you after surgery. In addition, the film crew may want to interview your family.

BY COMPLETING THE FORM BELOW, YOU ARE REQUESTING TO NOT BE ASKED TO PARTICIPATE IN THE ORGAN DONATION AND TRANSPLANTATION DOCUMENTARY PROJECT BETWEEN BARNES-JEWISH HOSPITAL, WASHINGTON UNIVERSITY SCHOOL OF MEDICINE AND A&E CABLE CHANNEL.

If you decide not to participate and complete the form below, your decision will in NO way impact your status on the waiting list or your potential for receiving a transplant.
