High Reliability: Every Patient, Every Person, Every Moment

Health care organizations are beginning to adopt the concept of high reliability, which many industries such as the military, airlines and some manufacturing industries have focused on for years. High reliability organizations operate under very trying conditions constantly yet manage to have fewer than their fair share of accidents.

To explore the concepts of high reliability within health care, Barnes-Jewish Hospital, Washington University School of Medicine and St. Louis Children’s Hospital have sponsored an annual Patient Safety &Quality Symposium for the past six years. The theme of each Symposium has been some aspect of high reliability. Speakers from across the country present on a multitude of topics all focusing on achieving and sustaining high reliability.

At Barnes-Jewish, high reliability means “every patient, every person, every moment,” specifically:

  • Achieving the best possible outcome for every patient
  • Engaging every team member
  • Delivering both of these every moment of every day

In 2015, we continued our journey of building stronger processes and practices for standardized delivery of care. The mantra of “Every Patient, Every Person, Every Moment” has been discussed thoroughly with leaders at the hospital’s ICARE Leadership Institutes throughout the year, which involves more than 500 leaders in all areas of the hospital. As we move forward, we will continue to engage leaders, team members, patients and family members to achieve exceptional care and improved outcomes.
